A free printable neighbor contact sheet where people share only what they choose. Build a simple street directory that stays on paper and stays private.

How to use it

  • Share is optional. Everyone fills in only what they are comfortable sharing.
  • Leave any box blank. A name alone is fine.
  • Adults only. Do not write children's names, ages or schools.
  • Keep the paper sheet private: do not post it, photograph it, or leave it where visitors can read it.
  • Hand out copies only to neighbors who signed up, and ask before adding anyone.
  • Offer to take a name off whenever someone asks.
